Local nonprofit receives $121K grant to advance health equity on Long Island
Options for Community Living has recently been awarded a $121,000 grant from the Health Equity Alliance of Long Island (HEALI) Social Care Network, to be put towards strengthening its efforts in addressing the social determinants of health affecting Long Islanders in need.
The funding will support critical infrastructure improvements in screening, navigation and care management in order to better facilitate connecting people with essential services and resources. The grant is part of HEALI’s broader initiative to empower community-based organizations with the systems and tools needed to better support underserved populations.
“We are so appreciative of this opportunity to deepen our collaboration with HEALI and area providers to further support people in need and help close the loop on gaps in care,” said Allison Covino, director of ATC Services at Options for Community Living.
In a statement, Options said the improvements for which the grant provides will enable it to increase reach, improve efficiency and foster stronger collaboration with partner organizations.
